Only Episode 6 has a episode name while Episode 5 show the file name of the video file.
Episode 6 is encoded in MKV while Episode 5 is on MP4. Actually All MP4 TV shows is not naming itself with the correct episode name only it’s file name.
Try going to the file itself on the computer, right click and select properties. Go to the details tab and if there are any names in it delete it, save and exit. Test 1 of them to see if it works, now goto PMS and go to the folder where the shows are housed and Scan Library Files. See if it fixes itself. If not what is the name of the file itself as an example?
Are you sure the mp4 based episodes actually get metadata or have you configured your library to use embedded metadata (e.g. title tag in mp4)? Plex is only using embedded metadata from mp4/m4v files – e.g. not from mkv!
General rule of thumb… get your file names fixed!
This is particularly important for tv shows where both Plex and TheTVDb.com are super strict.
Naming example:
TV Shows ← the folder linked to your tv show library .. > The Gifted .. .. > Season 02 ← preferably 2-digit season numbers .. .. .. > The Gifted - s02e05.ext ← ext referring to your file extension .. .. .. > The Gifted - s02e06.ext .. .. .. > The Gifted - s02e07.ext .. .. .. > ...
Remarks:
You can include the year the show first aired if you e.g. expect future shows with the same name (or if there already are shows with the same name) → here: The Gifted (2017).
The name of the show in the episode file name should match the show’s main folder, so if you used The Gifted (2017) there, the episode should be named The Gifted (2017) - s02e05.ext.
I use Filebot to add the metadata, If you do torrents then it can automatically unpack, add metadata, and organize into a Plex file structure. Or you can do it manually as well
My naming convention looks fine . The issue is some episode get the episode name and some are not.
Ex : TV Series > The 100 > Season 5 > TVShowName.S00.E00.ext
In there, grab the line ‘Local Media Assets’ with your mouse and drag it downwards, so it ends up being at the bottom of the stack of active agents.

Settings - Server - Agents - Shows - TheMovieDatabase
Settings - Server - Agents - Movies - Plex Movie
Settings - Server - Agents - Movies - TheMovieDatabase
Just because you naming works for some or even most TV shows does not mean it will work for all. Plex tries very hard to match shows no matter how badly named they are but the fact is your naming is wrong.
You need to get rid of the periods (Except for actual name punctuation) and insert dashes at the correct places. Also Plex does better with two digit season numbers for the season folders.

------------------Season 01 <— This folder holds all the episodes for season 01

This repeats for each show and you can have more than one “TV Shows” directory but the library needs to point at each of them.
Note: The year after the show name is only normally needed or desired if there is more that one show with exactly the same name. Check http://thetvdb.com/ if you are not sure and match exactly the name used there.
There are naming methods other than the above that work but this does work 99.9% of the time.
If you are going to use Plex it is best to use the proper naming conventions so Plex makes the best match possible. Trying to force Plex to use other naming methods is just asking for frustration.
No issue with the naming convention of my files it just the agent settings in PLEX. I already solved it. Thanks for the info also. Moving forward i’ll name it as you instructed. Will have to change my SONARR settings for it to auto rename all my tv shows. Thanks appreciate it.
