I recently swapped one of my RPi -> RPi2 and after installation, I'm unable to see the RPi2 on my Android App to control. Any suggestions?

In PHT under Preferences > Service; do you have "Allow Control of PHT from Plex Clients" checked?
Interesting....
I went into the menu, saw it was checked... attempted to uncheck and it crashed/rebooted... when it came back online, it was there...stupid me didn't note the bugid like it told me to... next time I think "I'll remember it" ... I'll write it down anyway
