Sega Genesis files should have an extension that is .gen for them to show and register in Plex Arcade. Correction: as described below .md files are the correct extension.
AFAIK, .gen files are basically an image file, similar to iso files. These are not supported. You need only the game portion which should be in a .md format.
Ok. I did have all my files in .Gen and they worked but I will change them all to .md…They all still work.

yes, they did. I used the cores that the core manager automatically downloads
If they work, then there is no need to change them. I didn’t think PMS would see the .gen extension.
the ones they didn’t see were .smd files which were what most of the free rom downloads seem to be.
a simple rename from smd to md should solve any issue there
