FWIW I also had this issue and was able to fix it by resetting the permissions on my transcode folder.
chown -R plex:users /path/to/transcode

Please do not put 777 permissions on anything.
Look at the owner of the directory.
ls -al /path/to/transcode
My issue was I was not the owner. The permissions were OK.
chown -R user:group /path/to/transcode
This has been happening to me for over a month. Attached are my logs. I am on the current version. Version 1.16.1.1246. It is random, different devices in the same house. some work, others dont. 1 Ipad works, other does not. I have uninstalled, reinstalled. Installed on a new drive and all still the same. Doesnt matter if its transcoding off my cpu, or off my P2000 GPU the same people on the same devices have the same issues. I even had them uninstall and reinstall.

I have had the same problem for a long time and havenāt been able to find a solution or get any help here on this forum.
My PMS ( Version 3.106.2) runs on a Qnap TS 253 Be NAS (Firmware 4.4.1.0978). Both have been updated regularly but the problem persists.

what frustrates me the most being a Plex pass Purchaser, is that there are so many people having this exact same issue and after reading through the comments, the response from a Team member is that they need people to provide info on the issue so they can figure it out⦠well hey I see 446 comments to this thread⦠and im sure its been posted 100s if not 1000s of times in other threads and it has not been fixed, let alone even brought up in any change logs. This has recently started happening on a few movies on my QNAP Plex Server instance. It only seems to happen on certain movies and usually at the beginning. Iām not sure how to provide logs. If someone can let me know how, I will.

Most of these are different. It is a generic error message for any transcoder failure. So each case needs to be investigated with logs and sample. Sometimes one cause is identified that is seen to affect many users - eg the change in MalwareBytes Premium which caused the transcoder to fail
Looking at the logs which cover a 50 minute period, i cannot see a transcoder exit due to an error

These were for skips during playback for the Alita Battle Angel video
There were no transcoder failures that I can see within the 50 minute period covered by the logs
Please in future give the time of failure and what was being played back when providing logs
The transcoder reported errors appear to be when playing back trailers. where it is reporting that it is encountering invalid data.
This can easily arise if the system clock on the server is wrong as the interface to the providers that supply the trailers rely on accurate time and authorization for access is time limited

My Qnap NAS was set to manual time adjustment and was almost 5 minutes off. Setting it to automatic time update solved the problem

The solution is that it doesnāt work, get lost. Thatās all I see from the Plex community.
Why is logging so terrible with plex? Hereās an idea Iād like to criticize- plex optimize. Itās garbage and never works.
I had this same issue and got it sorted. Can you please do me a favor and troubleshoot it with me?
1- u in docker or not? If not in docker, I have a strong recomendation for it.
2- can you ls -alsh inside and outside of your transcode directory? it needs to be owned by users, and permissioned 755. https://i.imgur.com/ULdDDyD.png see how my /transcode directory is owned by abc in the usergroup abc? Thats just because its docker and that is the default user. The important part is that it is owned by something that can write to it and be chmodded to be able to be written. This is SUPER common of a problem in linux.
The second issue it could be is your ffmpeg. That is the library used to transcode. If you have a bugged or out of date one, then it will throw errors. Malformed media will also throw errors. You can try a diffrent format or use ffprobe to scan for errors in the title. (btw docker has an internal version it installs, and thats why i recommend using it)

You may have found the problem yourself by now - the log shows the file does not exist

Has the drive letter perhaps changed ? You can use compmgmt to fix the drive letter - if this is an external drive
