I have a Media Server at my house, and wanted to watch my content at my girl friends house on her Apple TV. I had previous sucess doing this with the jailbreak on 5.0.2 using the Plex App, but now that is outdated and causing me problems. So I figured it was time to try Plex Connect.
You are correct you do not, you can use the same technique with port forwarding, however in your PlexConnect config, you have to specificy your public IP for the plex server and not use GDM. You will need to forward the appropriate ports to on your router as well (32400, 80, 53(UDP), 443). Also in your PMS configuration the easiest way to get it working is to add the remote location's IP under the "Allowed Networks" section.
I find that many routers these days come equipped with VPN support, so if you are confortable with setting it up I highly recommend it, as its easier and more secure.

That does work, pretty well actually, you are limited to stereo sound though
