I noticed when I try to add my TS recordings to my Plex Library that it will first create an item and then immediately delete the item. I then just rename the file and it recreates it just fine without deleting it. Here is one example from the Alerts Log for an episode of Survivor I recorded. This only occurs with recorded TS files and the rest of the media I add is fine.
I do the same using NextPVR and in this case as I was not creating an archive of the season but just place it my Video Files folder where I just watch the episode once and then manually delete it as the TS files are Too large for archiving. If I were to archive any of the recordings I would reencode them to Hevc.
I understand. That wasn’t the main point I was trying to make, however.
Your file naming and organization are extremely important for media to be recognized and for correct metadata to be applied. I don’t record Survivor, but here’s an example of a series recorded by NextPVR and added to one of my Plex libraries:

If your settings match mine from above, NextPVR should create the show/season/episode structure for you; you just need to place the whole tree in the library’s top-level scan folder (where you point your library).
For Survivor in particular, it may be useful to add the series’ release year to the folder name, in parentheses. For example, If I were to add it to the above it would be “Star Trek Deep Space Nine (1993)” instead of just the show name.
This will help disambiguate the show from other potential matches which might be causing duplication with already-existing shows.
I should have made it clear, the Files-Videos Library I am saving this to is set to “Other Videos” using the Plex Personal Media agent that does not use metadata. I never have this issue if I were to save the file to my “TV Series” Library which is formatted as you stated. Like I said it’s a weird one for why it deletes the item after it just created it.
