Im having the same issue. After months of great service. Why should it matter if XBL is up or down? Why won’t this app work locally. PLEX get your stuff together. An internet connection should NOT be necessary for me to watch movies on my own damn network. Both of my xbox one’s and my xbox 360 all suffer from this DESIGN FLAW. Should i even bother downloading PLEX on my ps4?
Agreed, first month as a paying customer, and this is not a good look. I'll give it little more time but I'm souring on the idea of paying for this.

They are finally showing it on the Xbox Status page as Limited.
This is still an issue on my XBOX 360.

Awesome. A couple of days ago I read about Plex on another site and I decided to give it a try. I didn't yet have the server installed or a PlexPass, but I installed the Xbox 360 app and it asked me to sign-in with a code (which I couldn't do obviously because I didn't have a PP yet). So this morning (the 30th), I purchased a 1-month subscription to see if Plex met my needs, but when I went to the Xbox app, I kept seeing the error everyone else has.
I too checked the Xbox site to see if anything was wrong, but nope, nothing was listed.
In addition to the steps others have mentioned taken, I completely formatted the device and ran the initial setup in case I had somehow mucked things up. Still nothing.
It took me forever to navigate these forums and I didn't see anything for the Xbox, then I realized I wasn't signed-in and after I did, I found this section and the exact issue I was looking for.

Works nicely, except that somehow I managed to rip my BD with subtitles embedded. Oops!
Come on Plex. Resolve this issue. This has been going off and on for months. It's very frustrating for my wife and kids who are not technically sound. This is suppose to make my life easier.
