From what I can see though your original file name did not match the actual show name it should have still loaded it with no metadata as the general format was fine afaict.
Since you don’t see anything it is possible it was matched as something you already have. Filter library episodes on duplicates to see. Then check Get Info for one of the episodes that have a duplicate and look if it has that show matched
File permissions could somehow borked but that is pretty rare on a Mac unless you intentionally changed things.
If not a duplicate I would do the Plex Dance then check out the server logs to see if the show folder name even shows up.
