Possibly related to subtitle issue here - PMS on Pi 4 with HDHomerun cannot play live TV?
Also make sure your HDHR’s H264 transcode feature is enabled in Plex’s tuner settings. I had a similar issue…although admittedly logs look different which was resolved by enabling the HDHomeRun’s H264 HW transcoder.
