An error occurred loading items to play on all items

Player Version#:4.146.0

Since updating to 1.41.7.9823 i noticed items would play fine but nothing shows up on Plex Dash. Even checked plex web and didn’t see anything “playing”. I could watch videos but they would never get marked as partially played or played.

I decided to restart my server, now everything throws “An error occurred loading items to play”. I verified the file does infact exist and i can watch it in VLC.
I have plenty of hard drive space.

Update: I rolled back to 1.41.6.9685 and I still get the same playback errors.
I also tried optimizing my database and cleaning bundles.

Update 2: my Nvidia shield still works, but my pixel 8 pro and browsers do not. Browser maybe a codec issue?

Update:

I installed EAC3 codecs onto my Windows 11 server PC. I didn’t notice any changes immediately after, but wanted to mention I did this.

I ran “Analyze” on my entire library. didn’t notice any immediate change on my local network.

I left the house and got my wife to try while remote (Pixel 6), it worked right away. I tried on my Pixel 8 Pro and it worked.

This morning I tried using plex for windows 1.108.1.307-dd5b87aa (different PC than the server), and I immediately get “An error occurred loading items to play.” on all videos, even ones that work on my phone.

I am currently using Brave v1.79.118 and i get the same error occurred issue (Both my PC and server).

It needs the codec installed as well, because it will be playing the file as-is. So the codec on the server machine won’t be used.

Try with a middle-of-the-road browser like Chrome or Firefox.
Brave is built to stop web apps in their tracks by default.

I installed the codec locally now. I did also try Firefox 139.0.1 and get the same error. Chrome is the same – but Brave is basically chrome and didn’t expect a different result.
I have been using Brave for about a year and never had playback issues like this.

I updated to the public release of PMS 1.41.8.9834 and still having the same problem.

  • Plex dash does not report me watching anything.
  • Items on Plex don’t show partially watched or watched, I must manually mark shows as watched.
  • watching on any browser results in An error occurred loading items to play.
  • I can watch on Android (both Pixel 8 Pro and Nvidia Shield), without watch status.
  • no difference between transcoding and direct play.

Here is more logs. First I tried to play the video on my browser and get thrown an error, then i immediately played it on my android phone and it worked.

I ran the database integrity_check and i get thrown an error:

I updated to 1.41.9.9912 and i am still experiencing issues. I know the changelog didn’t specify any changes, but thought it will still be helpful to report my issues are still present.

Here is a preview of my dashboard. You can clearly see there is local traffic (i’m playing a video) but it doesn’t show anything playing.

An update:

I am running Version 1.42.1.10060 of PMS.

I updated my Nvidia Drivers to Version 580.87

I added Plex Media Server.exe to my allow list under Malwarebytes

I ran ChuckPA’s DBRepair tool for windows, there were no warnings or errors, logs below:

I am still getting An error occurred loading items to play. on all videos when I play through my browser or Plex player on another PC. My Nvidia Shield and Plex for Android 10.26.0.2578 still works, but no history being updated on my continue watching.

I have tried Plex for Android 2025.22.0 and I get playback issues, so I continue to use 10.26.0. All videos throw an error they cannot be played, similar to the browser issue.

My watch history under My Profile > Me shows correctly, but only because i manually mark episodes as watched when i am done.

One thing I noticed is with Plex for Android (Tablet), that when there is a partially watched episode while watching offline, when it reconnects to my network, it will show as partially watched on my Continue Watching.

I am unsure where to go at this point. I would be willing to run any alpha or developer preview build to help figure out my issues.

Same issue and still no fix. I have been looking for a solution for weeks.

I was able to resolve my issue.

I found a backup from April 2025 and restored that. Playback now works in browser, partially watched episodes now show as partially watched, and the dashboard shows what is being watched.

I am happy to help any Plex Developer resolve the issue by comparing my April 2025 database with my September 2025 database. Please reach out here or DM.
