This should be making it so that when the xbox attempts to access my plex, theres the 4 digit code to re-activate the device.
But something is going on where its just automatically re-activating.
I am confident she doesnt know my plex password to reactivate the device, nor understands enough how to do so.
Are you sure you weren't logged into the account on her computer (like when you first associated the xbox)? If you were, when she goes to plex.tv/pin she simply has to reenter the pin on the screen.

Nope, I did it from my house. Over the phone..
Its a strange glitch perhaps, not sure how many users have tried to deactivate their device, then try opening plex a few times, it might take a few tries but it seems its reactivating it someone.. something, cached somewhere, probably.

Sorry about the delay, I don't have a 360 so I had to get someone else to test it out for me.
One of the employee's was able to repoduce and reported it internally.
I couldn't say all he replied to me was he was able to reproduce and reported it. :)
alright, so by reproduce it means it wasnt just me.
Hopefully once the trial period is over for plex pass users only, it will FORCE an update, because id rather not let someone have unrestricted permanent access to my server.
In order to disconnect the Plex for Xbox 360 app form your Plex account you will need to delete the "Single Sing-On Authentication" at the bottom of thePlex account page (https://plex.tv/users/edit)
