In my case, with the Apple TV 4K 2021, the only way it works without any audio/video delay is after doing the procedure wireless audio sync and also using the old player. The wireless audio sync must be done for each resolution that will be used.

Try turning match frame off in the Apple TV settings app for this content. This generally works to get audio in sync but introduces some judder. Depends whatever is the lesser of your two evils. So turn it back on when done.

Try the audio sync detailed above and you’ll have to do it for each resolution/fps/hdr setting you’ll use. 4K/23.9/Dolby vision, etc plus using the old player.

Can others chime in with their audio setup to see if there is a trend? I am no longer seeing this issue, and my setup is ATV4k (gen1) >> LG CX >> ARC >> Receiver. I’m awaiting a new Sonos ARC soundbar that will use eARC instead and I’m worried that this issue will pop back up.

I am not using eARC. My setup is Apple TV → Yamaha AV Amp → Panasonic OLED TV. Things are better since I followed the Old Player and other tips detailed above (thanks for those hints btw), but it is still not perfect. All other apps that display 4K content (Netflix,etc) all work flawlessly.
