I hope you’ve recovered and I’m sorry if this has been asked before but would it be possible to add audio delay +/- and subtitle delay +/- to the Apple TV app? I believe it exists for web(pc) app?

Just to pile again on this, swapping to the old player fixes the audio sync issue (at least as far as I can test) however 4k content does not start at all.
@DaveBinM are you guys looking into this issue?
I think it does exist already for subs, but I think it’s only for external text-based subs. We’re actively looking into the AV sync things at the moment. I can reproduce it with some samples provided, and our developers are currently investigating it.

I often notice a slight shift in audio sync after a while. No subtitles. Typically direct play 4K.
Is related, or is the problem only with subs?
Hi all, recently switched to Apple TV 4K (2021) from an Nvidia Shield. I also get audio sync issues in Plex, but only seems to be 4K movies that have the issue. Every one I have tried so far has the issue. Not had any issues with 720p/1080p content. Never had any similar issue on the Shield TV. Trying the “old player” in Plex made no difference for me, so currently don’t have a workaround. All other apps appear to be fine. Fingers crossed for a fix soon. Thanks.
In my case, with the Apple TV 4K 2021, the only way it works without any audio/video delay is after doing the procedure wireless audio sync and also using the old player.
The wireless audio sync must be done for each resolution that will be used.
Try turning match frame off in the Apple TV settings app for this content. This generally works to get audio in sync but introduces some judder. Depends whatever is the lesser of your two evils. So turn it back on when done.

Try the audio sync detailed above and you’ll have to do it for each resolution/fps/hdr setting you’ll use. 4K/23.9/Dolby vision, etc plus using the old player.

I too have been seeing the exact issues described in this thread. I have a 2017 ATV 4K plugged into a brand new LG OLED and then to a Sonos Beam Gen 2 over eARC.
I’ve calibrated the ATV with every video output mode that it supports so I can keep Frame Rate Matching turned on, and the audio sync is now spot on for iPlayer, Netflix, Disney+ etc… everything except Plex. I have Cinematic Motion enabled in the LG’s post-processing so I get some judder reduction without the Soap Opera effect but this does add a small delay to the video output, though, so calibration was required.
Plex remains the only app which is causing sync issues, and as have many have rightly pointed out, switching to the old player resolves the sync issues but introduces compatibility problems.
It feels to me that Plex’s advanced video player isn’t honouring the ATV’s sync calibration settings as what I’m seeing (and hearing) is very similar to how all the apps were behaving before I calibrated everything. For whatever reason, the old player does.
Can others chime in with their audio setup to see if there is a trend?
I am no longer seeing this issue, and my setup is ATV4k (gen1) >> LG CX >> ARC >> Receiver.
I’m awaiting a new Sonos ARC soundbar that will use eARC instead and I’m worried that this issue will pop back up.
I am not using eARC. My setup is Apple TV → Yamaha AV Amp → Panasonic OLED TV. Things are better since I followed the Old Player and other tips detailed above (thanks for those hints btw), but it is still not perfect. All other apps that display 4K content (Netflix,etc) all work flawlessly.
Apple TV 4k v2 → Denon 2500H → LG OLED here.
The sync problem seems isolated to 4k content and is a day 0 annoyance for me. Neither toggling between the old/new player nor vague tips above resulted in Plex doing what literally any other media player on the platform can do out of the box.
I’ve thrown money at Infuse to mitigate the problem on my end. Your move, Plex.
I was hoping that @DaveBinM might be able to give a quick update on this issue? Would be curious what current theories are, and whether there is anything more we can do to help?
The problem remains, and indeed, for some reason Infuse does not have this issue. A tip for those of you who shift to Infuse in the meanwhile - turning off the Chapter Selection skip option in settings returns FF/RW functionality to being much more like Plex’s. Ie, tap to skip forward/backward 10 seconds, instead of skipping chapters (which appears to be the default).
I don’t have any additional information for this at the moment, I’m afraid.
I have totally completely and utterly given up on Plex. Yes - I use Plex Media Server to manage my media but as a a client (media player) they simply fall short - BIG TIME! I have cancelled my Plex Pass as I really do not need it for just using the Server. I use Infuse for everything and simply cannot fault it. In my opinion - even the picture quality of Infuse is substantially better than Plex on my Apple TV 4k 2nd gen. I have asked quite a few people to give me their objective opinion - and everyone picks up that Plex player (other than the audio issues) has a much darker picture. Infuse is much brighter, clearer, especially dark scenes. Yes - I know Infuse is only Apple focussed so that limits it use - guess that’s why every room in my house with a TV has an Apple TV. Sorry Plex - I think you need to step up!
