I check this weekly and it started working for me this past weekend with perfect sync across a number of files that previously were completely out of sync, using new player, direct play, 1080p, varying frame rates just below 24 FPS, x265. Haven’t tested 4k yet.
I’ve been trying to reproduce this, and I used to be able to reproduce this trivially, but I haven’t been able to reproduce this since v7.22, when we bumped our version of mpv. Is anyone able to reproduce this on v7.22 or higher, and could provide a sample file?
Question: would the version bump of MPV also have affected sync issues on the Android version of Plex (on an Nvidia Shield)? Asking because I’ve had a devil of a time trying to get the audio sync in my home theater working consistently. No matter what I try, it always seems to be wrong, or slip from day to day. Now that you mention MPV, I’m wondering if this was a related issue that was showing up on the Shield?
My setup is: ATV 4K (2021) → Onkyo 676E → Samsung QLED 4K TV.
Over the weekend upgraded ATV to tvOS 15. Hoped it would make a difference–sadly not!
HDMI-CEC enabled - I switch my Apple TV on and my TV and AV Receiver all switches on at the same time.
Watch TV Series (all 720P MKV files). Using Plex on ATV there is a lip sync issue. Not big (less than a second) but just enough to annoy the living daylights out of me. It is so obvious I cannot focus on the episode. Infuse (same setup) works 100% PERFECTLY! Come on PLEX!!! Sort this out!
As mentioned in previous post, are you able to provide a sample file that doesn’t stay in sync? Just need enough so I can see if it’s sync or not. Generating Sample Files from Media | Plex Support
I am sadly having the same problem: Apple TV HD (not 4K) under the latest TV OS, Plex client and latest Plex server, 1.24.4.5081.6000, on a Synology DS1520+ under DSM 6.2.4
Symptoms: Audio is disturbingly out of sync but only on HD (1080p and 720p) content. The sync error stays the same throughout the length of a movie. Standard Definition files play OK. It is not the files or TV/ATV setup as if I view the same files using Homesharing from my Mac through the ATV then the HD files play OK, perfectly in sync.
I went to settings and changed to “Use Old Video Player” everything was back in sync.
My ATV is set to Match Content On
We’ve finally got some attention so let’s use it. Otherwise years of complaints will go unheard and you’ve missed your opportunity and will have no reason to complain again.

Just wanted to share that I’ve shared one video file with the sync/juddering problem, and came across another this evening. Happy to share if it helps, but I haven’t heard anything on the first submission. Please let me know if I can help any further.
One tidbit: video was juddering when running on Apple TV with Dolby Vision 4k, with Match Dynamic Range ON, and Match Frame Rate OFF. I just tried turning Matching Frame Rate to ON, and it currently works smoothly.
Note that Infuse was able to play the same file smoothly with the original settings (match dynamic range ON, match frame rate OFF). Ie, zero changes on the Apple TV, and the only difference was using Infuse.
I was able to replicate the AV sync thing, and have filed that internally to get resolved. I haven’t had a chance to look at the stuttering as yet (I’m currently in hospital), but it’s on my list as one of the first things to look into once I’m well enough to go home
For me the issue happens on basically EVERYTHING if I have Dolby vision enabled. As soon as I turn that off it’s basically fixed but still problem for me.
Upgraded to Apple TV 2021 4k. Same TV, cabling, speakers etc—
Major issues with audio sync with experimental player (default). I switched to “Old” player within Plex settings and issue is resolved.
Issue is only occurring in Plex app on 2021 Apple TV. Youtube, Netflix etc. are totally fine.
Just wanted to chip in and report the same thing. @RubberDucky1 your solution for using the old player has fixed it however, if the new experimental player is going to be the default going forward, this needs to be fixed asap.
I can’t say for sure but the new player is very unlikely to ever be the default.
There’s a totally unrelated but very recent thread regarding Dolby Vision in mkv on the Apple TV.
That involves actively triggering back to the older native player for those files even for users who currently run the new player as the default.
