I don’t think Plex Apple TV app is able to determine whether a certain option is enabled in the TV settings. Besides, Infuse doesn’t have this problem.
But how the Plex app interacts with 3:2 pull down, which is what the real cinema option turns on, is an issue with the plex app. 3:2 pulmonary causes the audio sync issues within the app.
Plex (or any Apple TV app) has no way of knowing, what kind of postprocessing black magic the TV performs. If TV does something with the framerate that requires additional audio postprocessing, then it’s the job of the TV to do that audio postprocessing as well. If the AV-receiver is not connected to the Apple TV directly, then this should not be an issue.

@sbrentn regarding the real cinema option, I think @stigger is correct, unless it actually changes the display refresh rate for the app. You can check that by enabling the “Debug Overlay for the Enhanced Video Player” in the app Settings, at the bottom of the list.
You will see something like Display FPS, and that will tell you what’s the refresh rate the app detects in your TV. If setting the real cinema option on and off changes that value, then I’d blame the app

@M_O_E I’m looking into your logs right now but according to those, you have the problem with the old player, not with the enhanced one (which is the one this thread is about).
Does it work fine for you if you use the enhanced player? As of today it should’ve been enabled automatically, but if not, go to Settings → Advanced and make sure Use Old Player is turned off.
I’m gonna check the sample file you provided with the old player anyway, but that one is a black box over which we don’t have much control, so not sure if we can do much about it
I’ll keep you posted with my findings.
It sadly doesn’t work with either player. I just switched to the old one with the hope that it would fix the problem:(
I tried your sample file with both the old and the new player (and I even got the original file you extracted the sample from) and everything works perfectly fine here 
I watched the entire episode and all worked fine. Do you have the latest version of tvOS and Plex Media Server?
Also is this something that happens from the very beginning or that “builds up over time”? (Like it works fine at the beginning but as playback goes on it gets worse and worse)
Have you tried in tvOS settings going to Video and Audio -> Audio Format -> and change the audio format to something else?

I also see the same problems as everybody else in this thread with audio out of sync. I have an Apple TV 4th generation with Frame Matching on. When playing 24p content the audio is out of sync with the new and legacy players in the Plex app. The only thing that really helps is to turn off Frame Matching so that Apple TV outputs 60p. I saw the same problems in the past in the Amazon Prime app with 24p content for a long time but finally they were fixed. It is clear to me that this needs to be fixed in the Plex app. Please keep working on it.

The issue I find with turning off frame matching is that 24p content isn’t completely smooth with video playback (about once every 10-15 seconds a few frames are dropped)
This is insane. The issue is clear. How can it take that long to fix it? The priority must be indeed low for this issue at plex

I’m also having this issue, it’s really a show-stopper for using Apple 4k/plex. Hopefully will be resolved :/. Can someone please summarise the best work-around now for this?
+1 Audio sync issue on my Apple TV 4K and the cause appears to be frame rate matching Setting on the Apple TV. Latest public software versions as of July 6, 2020.
My TV is a Vizio e601i-a3 which is not smart enough to adjust frame rate on its own so content looks better with frame rate matching turned on, but it throws Plex audio out of sync. I’ve also got a Vizio sound bar connected using HDMI Arc.
Disabling AppleTV frame rate matching fixes the audio sync, but then content may have stutter if the frame rate doesn’t match.
Just wanted to chime in I have the same issue with my ATV4. Audio delay with Enhanced player, resolved when turning off frame rate matching in atv settings or switching to old player. Without any changes the advanced playback info says the TV correctly pulls down frame rate to match (23.97). It’s clearly something iffy with the Plex app/enhanced player…
Please is anyone at Plex looking at this issue? I would hope it’s something solvable, other media apps work fine (Netflix hbogo)
Same problem here. Infuse plays in sync, but Plex audio is out-of-sync with the enhanched player. I would prefer Plex over Infuse (as a Plex Pass user). Automatic frame rate change is on. It’s an important setting for me as an European (= content in 25fps, 24fps, 23.97fps, 50fps, 60fps).
Still an issue for me…
