For me, it’s files with AC32.0 (direct play). AAC2.0 never is out of sync (direct play). It’s also files with Mp3 audio. It’s so random when it happens. Usually, I’ve noticed it happens is that when the next episode starts, you see Plex loading the movie (which is usually really FAST on direct play files (Roku Ultra)…it sort of stutters loading, when it starts playing (out of sync)…I pause/start and it’s back in sync…That same file that was out of sync could play perfectly fine next time…it’s something about the way the video loads up before Roku starts playing it. (for me anyway)
I have the same issue described here.
Roku Ultra, older 480p videos. Video doesn’t need transcoded, audio does.
If I drop the video quality so it has to transcode too it fixes the issue, but now it’s a lower video quality.
Has there been any update from Plex support?
I am having the same issue with recorded live TV. Audio starts out in sync but drifts throughout., it seems to be mostly triggered by commercials which Plex has removed. Is there any response from the Plex team? There are 122 posts here. Clearly there is an issue needing resolution or at minimum a response
While not ideal, if you have another plex player, give it a shot.
We have a roku that exhibits the problem.
When I play the same content on my Vizio smart TV’s Plex app, the problem is gone. The same audio transcoding happens, no video transcoding, just like the roku player.
It’s dumb, but we at least have a way to watch our older shows now.
Can confirm that this worked for me, too (and the only thing that worked). Disable Direct Play in the Roku app, but still allow Direct Streaming.
I have a Roku TV, using Roku wireless speakers, and playing some media files in the Plex app would have a noticeable audio delay. But it wasn’t consistent, as some files that were seemingly encoded the same way would have the problem when others didn’t.
I wish I didn’t have to do this, but the videos seem to play just fine and Plex’s website says “Direct Streaming a file uses very little processing power without any loss in video quality.”
I don’t regularly deal with subtitles, so I guess this is my solution for now.

I’m another with audio sync problems. For me it has only started happening in the last week on my Roku Streaming Stick+.
Frankly live tv on my Roku is unwatchable. I already suffered from the picture getting stuck and juddering (Roku update to 9.3.0 causes live tv video to freeze) and now add these audio issues to the pile.
Direct stream on the video and a transcode on the audio and after a min or 2 my audio will be completely out by a matter seconds, it’s not even marginal. Even if i force transcode of the video it will still drift out of sync… Very frustrating. I don’t have any workaround!
Is there a feature request for this that anyone can point to? The option to offset audio/video exists in many other clients from IPTV to other players. It seems after this many years it would have at least made it into the stream. Rather than devs or others suggesting 20 different ways that seem to work sometimes occasionally for some people, one thing that seems could solve it for pretty much everyone is the option to adjust the offset during play “somehow”. I’m wondering how moot hundreds of posts regarding audio offset and sync would drop off if this option was resolved programmatically. Don’t et me wrong I love Plex and have for years but as tech evolves and things like wireless speakers become even more standard, I think this is going to continue to be an issue. Arguably the question of “well why should we solve this?” or “Why can’t the speaker vendors or device manufacturers present an option form their side?” will come up a lot however with this big of an application fan base, why not throw us a bone? </2 cents>
Just putting my hat in the ring here. I’m also having the issue with FireTVs (both the original and the 4k stick) and Live TV. I haven’t noticed anything with any of my other media, but I am noticing it with Live TV and it’s really bad.
The total lack of response from @Plex is damning. Please don’t send me “offers” to subscribe for a Plex pass when you will not even acknowledge (let alone FIX) such a fundamental issue. UNACCEPTABLE in a paid product.
Was tired of having the audio go out of sync on my shield, and so wanted to see if there was any thread out there about it. Looks like once again, there is a long standing issue here. I have had audio sync issues for a couple years now with the web player, but never on a device like my shield or roku…
My shield plays direct stream always for video/audio both. This started recently on this, and it is infuriating. It only happens with livetv. I also seem to have more buffering off and on lately.
Does the plex team need diag logs when the audio is out of sync??
This is really not acceptable. The audio seems to get out of sync very quickly. We need a stable platform. Can we please just get plex to a state where there are no issues that affects so many people, before introducing more ‘features’?? I never had issues like this with the old windows media center… Why is it that plex can’t overcome things like audio syncing with its video… This shouldn’t be terribly difficult.
So… I will say this. I use FireTVs exclusively (both the full version and various Sticks), and I made one change:
Settings --> Video Quality --> Adjust Automatically = ON.
I made that one change and my audio syncing issues stopped completely. I understand that this might not work for everyone, but it’s worked for me across all of my FireTV devices.
Note that you MAY have to reboot your FireTV because the damn Plex app doesn’t actually quit.
Potential FIX
I was having the same issue with the PS4, Roku and Mobile App.
I switched the audio codec from ACC to AC3. This fixed the issue for me.
Hopefully that works for you.
Thanks but not really a fix. Potentially a workaround for some though. The majority of the recorded content where I have issues have AC3 stereo or AC3 5.1 only.

Roku Ultra playing recordings of live tv recorded through Plex via a HDhomerun connect4. Noticed the audio was very off, then while watching a movie I noticed it was slightly off and decided to consult google. The direct play disabled appears to have worked for the movie…haven’t tried the tv episodes yet. Again, this only seems to have manifested in the last few weeks.
Would also like to add my case in here too. the Web player through chrome has done this to me for years…
But recently my nvidia shield started having really bad audio sync issues. It is super frustrating. My roku devices don’t experience the issue. Plex support needs to get in here and figure this out.
I’ve been having this issue on my NVidia Shield for a month or two now. It doesn’t seem to happen every time, but it’s close to it. I’ve tried turning off passthrough and turning on the old video player. Nothing seems to fix it. Initially it seemed like pausing or fast forwarding would cause the issue but lately it happens regularly. I’ve given up using Plex on the Shield at this point. Emby works without issue and Plex works fine on my Rokus.
I believe we are having the same exact issue. If only Plex would comment on this to know at least let us know they are even on the forums?? lol. The only way a paying customer can get support…

I have exactly the same problem and I find it surprising (and VERY frustrating) that an issue this significant has been open for so long with no obvious response from Plex. Not impressed.
