Whenever commercial removal is turned on there are audio video sync issues. The audio is fine until the first commercial break is fixed. As more commercials are skipped the audio becomes more out of sync. Anyone else have this issue? How would I troubleshoot the problem?
I’ve had this issue for over a year now, I just turn off commercial skipping.

I did not have a chance to use my PC today, but I did try it with the iPhone app and it’s fine.
I am using Bluetooth for audio on the Apple TV. I’m going to try with the tv audio and see if that makes a difference.
I have the same issue. Playback on an Apple TV 4K. The only way to get it back in sync is to let it play out of sync for about 15-20 seconds after the commercial break. Then stop the playback and start again at the current location. I’ve seen a lot of people post on this issue, but have not found any solutions.
I do not have the same issue if I play content on my PC, iPhone, iPad or Amazon Fire.
I’m having the same issue. My library is on a NAS and plex server is running on an ASUS minipc w/ an i7 on the same switch. That switch is connected wirelessly (802.11ac measuring bandwidth of 200Mbs) to the appletv non-4K.
I’d tried optimizing the files to see if that helped, and it seemed too but it also burned the subtitles into my files, which was SUPER distracting.
And just to be sure, everyone is using the straight .ts files captured by the dvr, right? (no optimized versions or using a script to transcode into something else.)
Wanted to chime in that I’m also having this problem. In the early Plex DVR days, audio & video getting out of sync used to happen pretty frequently but somewhere along the line they got it sorted out. Then after they added commercial removal it started happening again, but they eventually got it sorted out. And now again it’s popped back up…I started noticing it not after the most recent server update but the version right before that. Stopping playback and then going back in fixes the issue for me, but it’s still annoying.
So, to add more information to the issue, I just bought a new 4k TV with an apple TV 4k to drive it. The issue still persists on the new TV, I can fix it by waiting a few seconds exiting the video, and then resuming from where I left off. That always seems to fix it. One thing to note, is that it doesn’t happen every time. It also doesn’t happen on my phone or PC. It does happen on both Apple TV’s the 4th generation and 4k versions. One is wired via gig ethernet and the other is on wireless AC, so it’s not a networking issue.
It does appear that other people are having this issue as well, so it’s not just my setup.

So I downgraded to 1.13.8.5395 and that seems to have mostly solved the issue. Since I did that (start of Nov), I’ve had the lip sync issue happen once with dvr’d content and not at all with non-dvr’d content.
(This is with commercial skipping ON and using the FiOS comskip config that is floating around on the internet.)
I’m having the same issue. The lag only happens when watching live TV and a commercial comes on. The video will start to lag, but the audio will continue as normal. It will stay out of sync until I hit FF on the remote to catch up. Sometime it works and sometimes the video and audio will get jittery after this. I usually pause for 10-15 sec, then start again.
Clients are FireTV Sticks and 1 FireTV Cube. And it only seems to happen on the Cube and 1 of the Sticks. My other 2 sticks seem to handle this fine.
I have commercial skip off. I turned it on to see if the config didn’t take, then turned off again. Issue still happens.

Update: I think this issue may not be Plex related. I think it’s the FireTV Stick and Cube. I decided to install the HDHomeRun app to see how the TV stream would work. This same audio jitter and video stutter during commercials happened there too. Also, after some more testing, this jitter only happened on cableTV channels, not local broadcast channels like CBS, NBC, ABC, FOX. So, I unpacked an old Roku3, and it plays the TV streams normally. Interesting that others see this same issue on more powerful devices like the AppleTV. Is there any difference in how a FireTV handles the stream vs. the AppleTV vs. the Roku?
