I have noticed that after the last server update for my PR4100 Plex server, shows are no longer automatically advancing to the next episode, sometimes they do, but most of the time they get “stuck” and I have to manually go to the next episode. Really annoying when you don’t have the remote right next to you. Noticed it seems to always occur with my daughter’s cartoons.

same for me if you found the solution I take 
Just want to add that my Fire TV version of Plex is 8.12.4.22902. I have cleared the cache and Uninstaller and reinstalled the Plex app to no avail. I I also noticed some glitches, when it won’t autoplay to next episode the previous episode is blank screened, I hit the home button and all of a sudden I have the audio of the next episode, but on the firetv homescreen, it won’t stop until I start a new app/show.
I have updated the Plex Server to Version 1.21.3.4046, but the issue is still occurring on my firetv. I even rebooted the tv and it is still occurring.
It also happens to me with same server version on Windows.
I do not want to criticize the work of others because I would not be able to do it, but I have noticed that for about 1 year the new versions of Plex always have some bugs that they did not have before, the most striking was the possibility of not deleting the elements from the playlists on android devices, hope you solve these problems. Thank you

After playing with older versions of the FireTV Plex app, I found that the release in December 2020, version 8.11.0.22186 does not exhibit the before mentioned autoplay issue. Reverted my install to this version and autopsy is working again.

As others have mentioned, auto-play-next functionality
is hit-or-miss, however, I have also discovered the following -
1.) If a show is reaching its end, AND
2.) I hit fast-forward, AND
3.) The jump would take it past the total run-time of the show,
Then. . . the app just stops playing, and sits on a black screen.
So, let’s say I’m watching a show with a 30-minute total run-time.
The credits are rolling, and now I’m at 29:15 into the show.
If I hit Fast-Forward/Advance 30 seconds ONCE, it will skip to 29:45,
and continue playing, as it should.
From 29:45, if I try to hit Fast-Forward/Advance 30 seconds again,
playback will stop, and the FireTV will sit on a black/empty screen,
when it SHOULD either take me to the next episode countdown screen,
or return me to the playback menu, as appropriate.
Perhaps the app is trying to fast-forward to a point too far into the future
and getting confused. . . or maybe it is hitting that same point that other
folks are encountering, when it won’t advance to the next episode automatically.
