I have an h.264 MP4 movie that buffers constantly on my iPhone and Apple TV but not on my PC. under the dashboard it says audio and video are direct stream. Seems weird.

I’d say remux it.
Just drag the file into MKVtoolnixGUI, ‘Start Muxing’.
Then put the file into your library and try again.
