If an episode with chapter metadata is imported and then is replaced with another file for the same episode, the chapter metadata will not be updated and remain the same as for the first file.
Did you let Plex re-analyze the files?
The episode shows the loading icon and all the other metadata is refreshed correctly to the new file’s except for chapter metadata. If I delete the file entirely and have plex realize the episode is gone, then add the new file in then it works correctly. So it’s just a problem with replacing the file for me.
Repeat the test, but wait at least for one night. So that the regularly scheduled saerver maintenance has been performed.
I’ve had one which has been on the server for 4 days and it still has the old chapter metadata. I do have chapter thumbnails on in case that matters.
Then I can only recommend to use a slightly different file name when replacing files.
For match-able items this should be no problem.
For completely custom media though, a Plex Dance is currently the only solution.

Are you saying that this only happens with tv show episodes, not with movies?
I haven’t tested it with movies.
