It only seems to happen with the New Player enabled, and it doesn’t happen for every video, but if you leave a large playlist playing on shuffle overnight, the Continue Watching list will start to accumulate them.
There is a bug where watched items are not getting marked properly. It is being worked on.
This sounds like the second issue I have in the thread below. Are you able to confirm? If so I can clean up my thread so it’s only got 1 issue in it.
