Cover Images Disappear

I added some educational videos from YouTube as a custom TV Series and downloaded posters from the web. I add the poster to the video and it works fine - for awhile. But eventually the poster disappears from the content and is not available to select under Poster. Any idea why this might be happening and a fix? Note that

Would you mind elaborating your use case a bit?
Specifically… what do you mean by

Are you adding the poster as local media asset alongside the video file, or are you adding it via the Plex user interface?

I am adding it via Plex user interface. I am not keeping the poster file with the video file. Here is an example:

The poster image shows on my server but does not appear on any of the players:
