I have the same issue.
If the file takes too long to start up for some reason then it will revert to transcoding.
It happens after the file has been direct playing for a significant amount of time. Up to an hour sometimes before it fails then falls back to direct stream.
Do you have the app and/or server logs right after it fails
I’ll try and get them now.

Fixed my issue. The HDMI cable between the Receiver and the TV was causing it. Was meant to be a 2.1 HMDI but something obviously wrong with it. Got a new certified 2.1 HMDI and the issues are finally gone.
Was causing the audio and video to drop sporadically which I assume tells the plex server that it can’t handle the file in that package/format.
