Are all of these using the same client?
For the failed audio transcodes, you could try deleting all files from the codecs folder. Plex will automatically re-download all of the codecs it needs. To do this, find your Plex data folder. In the data folder there should be a sub-folder named codecs. Stop Plex and delete everything INSIDE that folder (don’t delete the folder itself.) Then start Plex. I had trouble finding a thread of this for win, but here is one for Linux.
