After updated to 1.25.2.5319 from 1.24.5.5173 on macOS, DVR recordings are failing and trying to watch live is failing with the error “Error code: s1001 (Network)”.
Sometimes it will play successfully after several attempts, but it is failing more times than it is succeeding.
I rolled back to version 1.24.5.5173 and playback and recording started working as expected.
I also tried installing version 1.25.1.5286 instead (which I believe is the version previous to 1.25.2.5319) and I still have this issue, so it doesn’t look like this issue started with version 1.25.2.5319.

I think I’ve solved this myself.
I had tvhProxy configured to use the default pass profile (MPEG-TS passthrough) in TVHeadend.
After adding a new MPEG-TS/av-lib profile to TVHeadend and configuring tvhProxy to use this, it now appears to be working even in the latest version of Plex at least for live TV. I have some recordings scheduled for today so I’ll know later if those are also working.

While I am using HDHomeRun Prime as tuner, I have similar problems that also seem to point to a change introduced with 1.25.x.xxxx. Live TV and DVR functionality was restored when I rolled back to 1.24.5.5173.
UPDATED: 2022-JAN-02: Rollback did not resolve the problem. Server Logs available via PM.

When using PMS version 1.25.2.5319, PLEX and the HDHomeRun software (Mac) cannot maintain a connection to the Prime’s tuners. After restarting the Mac, Plex “Live TV and DVR” service works normally and so does the HDHomeRun software (Mac). I am able to tune channels, set recordings etc.
However, after a period of time (ranging from minutes to hours), both Plex and HDHomeRun seem to lose their connection to the tuners. Selecting a channel in Plex results in an error “Playback Error Could not tune channel. Please check your tuner or antenna” and rescheduled recordings fail.
The HDHomeRun software opens to the “Recordings” tab but with no other tabs available. If I try to use “hdhomerun_config [deviceID] set /sys/restart self” it will report “no device found”.
I also noticed that when the tuners are lost, the SpeedTest app will fail to locate a nearby server and report that the Mac is not connected to the internet. However, I can access and use the Speedtest via Ookla’s website as well as Fast.com. Its the strangest thing.

It seems that the issues I was experiencing were due to SOPHOS Home.
I contacted SOPHOS HOME Support on 3 December. They advised me to add “web exceptions” for the local loopback address (127.0.0.1) as well as for the local IPs of the affected devices (in my case, the HDHomeRun Prime and the Plex Media Server).
After a re-start, DVR Recording and Live TV are working normally since Friday 7 December.
I have the same Freeview, tvhproxy and tvheadend configuration and am seeing randomly that some recordings and/or LiveTV are sound only and when I check the logs I see

I’ve just tried what you suggested now and will see how I get on myself…
It did resolve the issue, but I sometimes found that I would get occasional dropouts in the recordings (video and audio) possibly due to the conversion process.
I’ve now created another new profile of the type “MPEG-TS Pass-through” which is the same as the standard pass profile but with these options turned off (the view level needs to be set to Expert to see those options):
Rewrite PMT
Rewrite PAT
Rewrite SDT
Rewrite EIT
Those options rewrite certain packets in the stream to only refer to the currently streamed service, which looks like it may cause issues with Plex.
Plex appears to be working with this profile, and I haven’t noticed any dropouts but I’ve not recorded much since making this change.
The av-lib profile was based on a suggestion I had seen elsewhere, but can’t remember where now.
The modified passthrough profile was really just trial and error. I just decided to create a custom profile based on the existing pass profile and experimented with what changing different options did.
