I’m leaving this for others who may run into the same issue. I couldn’t figure out why the subtitles were multiple seconds early when playing a video on my AppleTV (App 8.45) when the same video played through Plex on my Mac was well synchronized. I found that my AppleTV app had “Use Old Video Player” enabled - disabling that toggle seems to have fixed the issue.
I have the same issue. Problem is when deactivate the old player. Some files don’t play (spinning indefinitely)
And all these files play perfectly on iPhone, macOS Plex or PlexHTPC or KODI (Plex- Addon).
I have tried many different settings suggested by many and it’s always the same problem.
I have installed latest server and now subtitles of all movies and tv series out of sync no problems of PC or iPad switching old or new player doesn’t help.
I have managed to get the subtitles working in sync on Apple TV (and also view some shows which would not start up) by changing the Plex setting in Apple TV (near the bottom), under SUBTITLES, ‘Burn Subtitles’. To ‘on’ instead of ‘automatic’ I am running latest server version and latest version for Apple TV (8.45).
I Have rolled back to the previous version of Plex server and all subtitles are in sync again. Il wait for a new release of the Plex server and try your solution if the problem occurs again. But to me it seems a problem in the plex server introduces in the latest release
This is happening to me too and the workaround seems to be always burn the subtitles

Oh wow, this worked for me. Finally! Thank you. The options I had were Automatic, Always or Only Image Fornats.
Switching from Automatic to Always resolved my subtitles suddenly being out of sync on all video.
