I run a media server from a Mac. If it’s closed/offline when I start Plex on the Fire TV then I’m unable to connect to it when I open the computer to bring the server online. I have to shut down Fire TV and then open Plex again.
Essentially, the server has to be online when Plex on the Fire TV is started otherwise there is no way to connect.
There doesn’t appear to be a way to do hard refresh on Plex without shutting down Fire TV.
It’s possible the Plex app is still running in the background on the Fire Stick when you think you have exited out of it. If you simply hit the Home button to jump out of it the Plex app can remain active (you can tell since re-entering the app takes you right back to the library/pre-play screen you were on before). Try hitting the Back button repeatedly to exit out of the app, then wait a few seconds before you try re-launching it.
