I had been successfully running plex for over a year when about 3-4 months ago I ran into issues. It started with library update issues resulting in titles being removed from my library and later being re-added, which resulted in the recently added section showing movies that had been in my library from months ago. It now has resulted in Roku streaming errors and general issues running plex interface on the roku. Everything is hardwired via ethernet into the same Dell PowerConnect Gigabit switch.
I have a Synology NAS as my storage and a Late 2012 Quad Core 2.3 GHZ with 16GB Ram mac mini running my Plex server. I have the synology mounted as a network drive on the Mac Mini via AFP.
It sounds like your computer is losing it’s network connection to the Syno. If PMS doesn’t have a stable connection (the media isn’t present/can be connected to) when it scans its library, it will be removed. Later, when the connection is restored, it is added back.
I would need to see your Logs from a situation when/as this happens. Asking about it 3-4 months later is of no help.
If you’re having streaming errors AND your media has been frequently removed and added back, it’s very possible your database has become fragmented. This can result in very erratic behavior. A simple “Optimize Database” (left side panel, top) will correct it.
If this does not, please gather the Plex Media Server logs in a zip or tar file and attach them here with with a post and we’ll take a look
I have found some errors. If you look at “Plex Media Scanner.log” you’ll see where it had different counts. It had tried to match 18, but ultimately wasn’t able to lock (80%+ certainty) On subsequent scans it lost that ‘lock’ and it dropped out.
A pass using a tool, such as FileBot, will help you put everything in order and this will be behind you. I recommend FileBot because as you drop in files to be renamed, it performs the lookup, constructs a proposed name, and allows you to verify this is correct before actually performing the renaming. If it can’t get a solid match from the names you’ve supplied, it asks. This is key. Since it shows you the resultant file’s path for each, you can verify everything is named as it should be for Plex.

Here’s an example of how FileBot can use both the current structure (to give hints) along with the name, and come up with matches. At no point did I have to help it.
To recommend the renaming rules for you to use in FileBot (Right click on ‘Match’, select “Edit Format”, switch to “Episode Mode”) which matches what appears to be your established theme,

You just enabled security after having used it all this time?
You can select “on same network” to continue. If your PMS exists on wired but Roku is on wifi with a different subnet ID, this can happen… Simply select “Always” after trying “On same network”
