Issue With Video Playback on Plex (PMS Running off Mac-Mini with files on a DAS)

Hi All,
Experiencing funky/frustrating playback issues with my Plex server.

Symptoms & Behaviour:

  • Impacts TV Shows & Movies, but mostly TV Shows (seems to be recently added shows. Stuff added pre April 2022 generally seems fine).
  • Impacts all video formats (i’ve tried swapping out video formats for content)
  • Content sometimes stops playing and kicks me back out to to the main Plex Menu (after about 5-10 seconds)
  • sometimes i’ll see weird pixelation occur (taking up whole screen)
  • sometimes i’ll see screen turn almost entirely green and also pixelate
  • Sometimes audio will either outpace video, or lag behind it severely

Troubleshooting:

  • I’ve seen this behaviour impact playback on: Xbox Series S, 2020 Roku Ultra, 2018 Ipad, as well as via Web Browser (across multiple computers with entirely different hardware and across multiple browsers).
  • Have tried swapping out these video files for different video formats (sometimes playback requires transcoding, other times not) to the same result
  • Playback of these video files on the Plex Server itself, via VLC, plays absolutely fine without issue
  • Have Optimized Database (Under Plex > Manage > Troubleshooting)
  • Under Settings > Transcoder, I have both “use hardware acceleration when available” and “use hardware-accelerated video encoding” check-marked

Environmental Details For Server:
V* ersion 1.26.2.5797

  • PMS is running off a 2018 Mac Mini (MacOS: Monterey 12.3.1).
  • Hardware: 3.2Ghz 6-core i7, 16GB RAM 2667MHz DDR4 & Intel UHD Graphics 630 1536 MB
  • Storage: 2-Bay QNAP DAS with two 3TB Drives

Any recent environmental changes?:

  • The* HDD’s in the 2-Bay QNAP DAS did get replaced in April 2022 (Retired 2 4TB Drives and replaced with 2 6TB Drives). Drives are: Seagate ST6000VN0033 Iron Wolf Multimedia Server Storage 6TB Internal Hard Drive 3.5" - SATA

Any idea what might be causing this? Or how to resolve?

Did run Disk Utility Analysis on the DAS on MacOS and go this:

Would best bet be to back-up data and repair drive?

Note: HDD is not being used for anything other than storing media files for Plex (no Dual Boot uses, etc..)
