Just got Roku Ultra. Cannot get it authorized by my media server

I just got my second Roku Ultra. I installed PLEX and linked it using https://plex.tv/link many times. I cannot connect to my media server. From Roku when I go to PLEX >> User>> Media Server Status, it says my device is Unauthorized.

However, PLEX server indicating my device is authorized (referring to PLEX BSMT in below screenshot)
