I have the same problem. It keeps failing 9 out of 10 times for me too like Sam86 explained in a previous post. This is a popular feature, so I suppose it will get fixed pretty soon. In any case adding more subtitle agents might add some redundancy in case one gets overworked.
PLEX, Think we need you to comment on this. Is something wrong or are we doing something worng. As it wirks now it is not usable at all.

Disabling my VPN fixed it for me as well
Looks like OpenDNS blocks this. You need to allow dl.opensubtitles.org and opensubtitles.org.
I have this annoying problem too; it simply does not show me all available subtitles on opensubtitles website. Please see two screenshots attached. Any ideas or solutions?
Not sure I understand. I do not use OpenDNS, at least not to my knowledge.
