Plex apps for iOS not working on iPhone 12 pro max + T-Mobile 5G + WireGuard VPN

Server Version#: Version 1.24.0.4930-6000 on Synology
Player Version#: plexamp for ios version 3.5; plex for ios version 7.21.1

The plex apps don’t work on iPhone 12 pro max with T-Mobile 5G service. I use a wireguard VPN to connect the phone to a server in the cloud. There is routing to and from the VPN and my home LAN (the server has no special config to access the vpn, as the networks are route-able). Other apps that access the home LAN work and I can connect to plex using safari. Furthermore, the apps work on my iPhone 11 with verizon (nearly identical setup). Seems like a bug. The issue looks very similar to this closed one:

Server Version#: 1.22.2.4282 Player Version#: Latest iOS App I have CGNAT on my iPhone 8. However I have setup a wireguard tunnel that reaches the network. I am able to open safari hit the ip:32400/web and look for content and start a stream However when I go into the app. I can see the media, explore the libraries, but when I click on play it never starts streaming. I can access other network resources on via the wireguard tunnel no problem. Any ideas what I need to do?

Edit: Forgot to mention, I’m not using NAT between the VPN and LAN. The plex server sees the VPN IP of the iphone 11. With that phone, I can see the stream in the “NOW PLAYING” section of the online dashboard and it works great. Not sure why it doesn’t work on the iPhone 12

I have the same issue on an iPhone 11 on T-Mobile with a WireGuard split tunnel. I can only see the Plex server explicitly routed through the WireGuard and not my other remote server. I assume it is a split tunnel iOS issue but just wanted to chime in.
