The plex apps don’t work on iPhone 12 pro max with T-Mobile 5G service. I use a wireguard VPN to connect the phone to a server in the cloud. There is routing to and from the VPN and my home LAN (the server has no special config to access the vpn, as the networks are route-able). Other apps that access the home LAN work and I can connect to plex using safari. Furthermore, the apps work on my iPhone 11 with verizon (nearly identical setup). Seems like a bug. The issue looks very similar to this closed one:
Edit: Forgot to mention, I’m not using NAT between the VPN and LAN. The plex server sees the VPN IP of the iphone 11. With that phone, I can see the stream in the “NOW PLAYING” section of the online dashboard and it works great. Not sure why it doesn’t work on the iPhone 12
