I can repro the issue with your file, I see it only uses the embedded artwork when I explicitly set the library to prefer local metadata which shouldn’t be needed.
I’ll have to see why my sample file didn’t have this behaviour, I suspect it might be something to do with the ordering of the images being returned but I will check and fix this.

I’m having a similar issue with my Various Artists folder. It’s ignoring the generic album.jpg in main Music folder and Various Artists folder. I’m expecting Plex to use album..jpg whenever there is not an album cover stored in the album folder and for individual tracks not in album folder that are missing embedded album cover.
Notice that in the above “Hardy Hixtape - Vol 2” picture there are 3 album names, even though as shown here they are all the same. And they are using the same randomly picked image as with the other issues. Note that after performing Refresh Metadata multiple times, the correct album artwork replaced the generic one.

Some show [unknown album], even though Album Artist of “Various Artists” is embedded in the metadata, but without album name. Also, why are there multiple [unknown album]?
Rather then all tracks from the same Various Artists album showing in the same album, they are randomly separated into albums using the same album cover and name. See the “Hardy Hixtape - Vol 2” in above pic.
Did the same with “iHeartRadio Living Room Concert” as shown in the screen shot for item 1 above.
Randomly grabbed all tracks, that had no embedded album name, but had Album Artists of “Various Artists”, from “\Server3\music\Various Artists\Old Time Salsa Hits” to display within Various Artists “Unknown Album]” (notice the missing open bracket) that is pulled from “\Server3\music\Alabama\Angels Amoung Us”. Only the first song is from Alabama.
Once I added the album name to all the Salsa songs, it fixed itself, but still missing the opening bracket. It also used the same random album cover for this album. Refresh metadata made no difference.
I ran into the same issue. Since I set up my music library I exclusively use the embedded artwork and have the scanner accordingly configured. All my files are carefully tagged and hence the correct artwork is also very important to me. This also worked well so far.
Recently, more and more albums that had the right (embedded) cover suddenly started to show a different cover. Refresh Metadata doesn’t help. I can manually select the embedded cover from the poster section of the album, so the data is still there. During the manual refresh, the logs also extract the cover art and when I check the temporary file, it matches the cover I embedded.
So for some unknown reason Plex Server suddenly started to use the agent cover data instead of the embedded cover despite the setting to only use local cover art.
Thank you for the update. I hope 1.43.0 will fix the issue. My music library is gradually becoming less and less recognisable due to all the incorrect covers popping up (I usually select albums by cover…).
Will the update also take care that all the automatically changed cover associations are fixed back to the embedded/local artwork (and ideally that the downloaded covers are removed)?
The item would need to be refreshed for the change to get picked up. This should happen automatically over time during maintenance hours (unless you have that setting disabled).
