Plex recently gone crazy on me and started to group all seasons into season 1.
My format
Show name/season 1/show name s01e01.mkv
On the surface your naming looks to be correct. If you’re familiar with the Plex Dance, I would recommend doing that. Then attempt to re-add the show one season at a time. Out of curiosity, what show is it?
There is a setting in the properties of the library, which allows to hide all seasons. You probably have that set wrong.
No plex is grouping everything into season 1. It confirmed by going into info. All the seasons are listed in season 1 info.
The only way to fix it is ro remover the other season. Scan it and put the season back and scan it again.
Try using double digits for the season folder names, so Season 01 instead of Season 1 etc…

I have seen this behaviour before, did the same with Empty Trash in between. I always name my Seasons 01 - 09, 10 - 100.
Are the files mp4?
