95% of my music library is lossless (flac).
While Plex is otherwise running flawlessly, it insists on downloading image files rather than accepting the embedded image. “Use embedded tags” is checked in advance settings.

This is only one half of what’s needed.
Settings - Server - Agents - Albums - PremiumMusic/last.fm
Make sure the line ‘Local Media Assets’ is on top position.
That being said:
I recommend you to include a folder.jpg in every album folder with the cover art in the highest quality you can find. Embedding a high quality cover art in every file of an album is ridiculously redundant.
