Under Library Settings Advanced View there is no country selection option. However, under Agents and the scanners I have everything set to USA, still get foreign posters.

That is also under Agent configuration. I have it set to US and I am using the Movie Database agent for my library.
I don’t have the issue, so something amiss . They/foreign are in my poster galleries, but default Posters are always English.
I have noticed “Localized Titles” is not checked.
Found this in a discussion.

I checked it now, but please know that I am not using That Agent. Are you recommending I switch to Plex Movie Agent? That option is not available for The Movie Database Agent.

Well you can always go back, I’ve tried all agents and have settled down with what works best. Cast was issue previously now it much better.
You will need to update Plex and probably restart.

Under each tab, that Agent should be at the top with the ‘alternate’ agent under it. Local Media Assets should also be ‘demoted’ to the bottom of every Agent list - if you ever want to use MP4/M4V files. Plex will read and react (in horrible ways) to bogus embedded Title Fields in MP4/M4V files.
I do sometimes get a non-english poster, but not very often.

The option “Prefer Artwork in library language” is present in both agents. If it is ticked, the posters will be selected according to the language that is picked for the library.
You should verify what language you have set your library to.
‘Edit’ the movie library, the language selector is right on the first ‘General’ tab.
