Probably have a deletion policy on library show settings. Take a current show episode, rename to match show you are trying to record and put it in your library. Once Plex picks up the show edit the library settings for the show
Would need at least the Plex Media Server log files. At this point the log file has probably been overwritten. My logs only have about 8 hours in them.
Having trouble pinpointing the example above, however another show is doing a similar thing. I can’t even add it to my scheduled recordings. I found a log event that corresponds to the time when I tried to add the show and it failed with the same message as above:

Did you ever update Plex to the current code release? Are you still on 1.7x? I didn’t see it mentioned in this post string that you moved on from 1.7x…
I would try the much improved current version if you have not upgraded yet, and then retest…
I lost some recordings that looked like this - for me, it turned out there was a permission problem on the folder the recordings were meant to be stored in (i.e. the plex application didn’t have write access to the folder).
Not sure if it is the same problem, but it might be worth checking plex can write to x:\shared videos\TV_Adult
Really need the Plex Media Server log not the Plex Media Scanner. Log you submitted does not have anything after 10:41:28.778 so no entries at 10:42:36.983. Normally it is easier to download the zip file containing all log files and submit the zip file.
I have a similar problem, shows are saved in “.grab” but are never moved to the library when done. Plex shows them as successfully recorded but they do not appear in the library.
