Plex sees duplicate Music tracks when only one source file exists

Server Version#: Version 1.19.3.2764
Player Version#: Web 4.33.1 (but also experienced on Android)

(If I can pull any logs to assist with diagnosing this, please point me to a guide that explains how to get them, thanks.)

Issue: Sometimes I have music tracks that Plex sees duplicates of, and other tracks are just fine. I’ll include some pictures for reference. The tracks that Plex duplicates link together extending the track length and repeating the track for each number of duplicates that Plex sees. But there are actually no duplicate files, Plex is making them up, or they aren’t being cleared properly from Plex for some reason. “Automatically empty trash can” options is selected, and I’ve tried un-checking and re-checking, and also rebooting in between to see if it would clear old data, but it hasn’t helped.

I’m running Plex in a Docker Container on Ubuntu Server 18.04 LTS using the official Plex Media Server Docker Repo.
I have different media directories on my server, but they’re each mapped separately and nothing overlapping (my movies and TV are in /lib/e/ and /lib/b, my music is in /lib/d/music, pictures in /d/pictures, I did not map the /lib/d parent directory, so no accidental overlap/scans of the same directory twice).
One concern I have with music is that I have 23421 tracks, 1453 Artists, and 2559 Albums.
In 2019 I upgraded my server and started using Docker. I copied all my metadata/play history, etc.

I’d rather not reset all my play history, metadata and ratings if I can avoid it, but I’m thinking the best fix for this might be a fresh start with my music library.

Anybody have any advice or ideas?
