Do you have automatic or periodic library updates enabled?
If the file hasn’t been copied completely, the server might compute the “bogo hash” based only on the file header. Which is very similar for most video files, so it will mismatch any other partly-copied movie to the same wrong metadata.
- disable automatic and periodic library updates
- perform the Plex Dance with all movies involved in the mixup
