After updating Plex Server from 0.9.14.1 to 0.9.14.2 Plex on my ps4 won’t play films. It can see everything fine but thwn I go to play it loads the play screen with the progress bars and the blurred background with the black box and 3 dots but it never gets any further. I’ve tried lots of different files with no luck. The Plex on PS4 is the latest version.
Films play great on my other devices like the iPad and iPhone.
I think we are going to need media info and server logs to help you with this one. are you able to post them?

Roll your server back. I had the same thing happen. I noticed that the plex pass page was showing a newer server version, but the web app was saying I was up to date, so I went ahead and manually applied 0.9.14 and once I did that I couldn’t play anything on my playstation devices.
Rolled back to the non plex pass server version and I’m back in business. Seriously don’t mess with getting all that info in the post above, just uninstall current server and roll it back to 0.9.12 and your stuff will work again.
rolling back did the trick! should have thought of that really myself… cheers
Yep, we’ve just found the issue ourselves. Working on a server fix.
Posted known issue here: https://forums.plex.tv/discussion/195573/caution-plex-media-server-0-9-14-3-breaks-ps3-and-ps4-transcoded-and-direct-stream-playback?new=1
It’s now January 2016, any news regarding this fix?

I’m also curious as to what is the status of this?
Since the issue post which mattseeley added there have been further releases for Plex Media Server which include the permanent fix for this issue. Just ensure you are fully up to date with PMS and you should be fine.
If you’re having issues with playback of particular files I woudl suggest creating a new thread with an explanation of the problem and a set of log files.

Is this actually fixed yet? I have a Plex Pass and I updated again (3rd time in past week or so) to version 0.9.16.2.1827. I STILL cannot play video (.mkv or .mp4) on my PS3 or PS4 . Apple TV still works fine though. When I try to play .mp3 audio files, I get an error message stating “the video file cannot be transcoded.” Currently, I can only play .m4a audio files on either PlayStation. It should be noted that I’m running Plex Server on a Synology NAS. I would love a definitive answer here. Do we still need to downgrade to get PlayStation support back?
For testing purposes, I just tried to downgrade to public version 0.9.15.6.1714. The Synology package center would not let me downgrade. I don’t want to do an uninstall as I fear this will delete all my metadata and I have a very large music collection in Plex. Now what?
Another question…The PS3 has a built-in DLNA client. Why is it that I can stream and VIEW videos from PLEX server to PS3 via DLNA (using PS3 built-in client) but I cannot view the exact same videos inside the PLEX PS3 app? Are we sure this is just a server issue? It also feels like a PLEX client/app issue as well. There is definitely something wrong at the server level too, as I cannot play .mp3 audio files via DLNA client (appears as unsupported format). So the server is definitely doing something to make the PS3 think that .mp3s are not actually .mp3s.

There are different reasons for transcode to fail. As suggested by @DavidIrwin best is to raise a new topic with Debug logging from the server and the media info xml to get the problem looked into
See https://support.plex.tv/hc/en-us/articles/201643703-Reporting-issues-with-Plex-Media-Server
and Plex Media Server Log files
and https://support.plex.tv/hc/en-us/articles/201998867-Investigate-Media-Information-and-Formats
That would be a good start
