Quick question, I’m running PMS off of an old MS Surface Pro that’s wired to my router, and the files are on an external USB drive connected to the Surface…
For that setup, would it make any difference to connect the external USB drive directly to the router instead?

Why not just try it?
Because there could be benefits or disadvantages that aren’t immediately obvious. So figured I’d ask if there were any.
If you do it that way, you’ll have to rely on the router to be reasonably responsive. However, many smaller devices which can act as a file server, are not like that.
They can sometimes take a few seconds until they respond to client requests. This can already be enough delay for Plex server to give up and mark the file as “unavailable”.
