Quick update as I’ve been playing around a bit…
Technically I have two servers - the NAS and one on my Mac Mini that I use to play around with and test configs and so on (it is only running sometimes). I went in and played with some settings and enabled Relay on the Server(s). That has sort of helped as I can now connect to the NAS indirectly.
But here’s the weird thing - both servers are on the same network. If both are running and I connect to Plex Web through the NAS (at ravenloft.local), the NAS server connection is good (green lock) and the Mac Mini server shows as an indirect connection with a yellow exclamation mark. If I go through the Mac Mini (at 127.0.0.1.local) both server connection are good (and two green locks is what I would expect to see from anywhere on the same subnet). If I go through Plex at app.plex.tv/desktop - both servers are yellow exclamation marks showing as indirect relay connections.
And still, both Remote Access settings show the external port is set to 0 (which I suspect is a big part of the problem as: 1) port 0 cannot be directly accessed, so that is either an error of some kind, or something is happening that is being interpreted as mapping port 0; and, 2) with both servers on the same subnet if port 0 was actually being used then the public IP:Port combination for the two servers would be identical, so how would Plex be able to manage those connections in the first place?
Fairly certain this is a key issue - but can’t see how to fix it, unfortunately.
The server is running on a QNAP TS-251D, on my home residential network. Remote access is set up manually with port forwarding on the router (although that was actually troubleshooting - as without port forwarding, the assigned port on the external IP address is 0 for some reason, not a random port).
As per the subject - remote access is mostly non-functional, although not completely. For most systems, attempts to access my Server via app.plex.tv/desktop end up with this condition:
This is true when accessing the server from macOS/Safari, the Plex iOS app and for immediate family to whom I have provided access to the server. However, I can still access the server from Windows/Firefox and from my local network, of course (the Windows machine is behind a VPN and not on the local network).

That said, the page flip-flops from stating fully accessible to not accessible constantly - none of which affects the actual accessibility as described above.
The only other recent change I am aware of is Plex’s decision to block certain hosting providers for rampant ToS violations - but as I said, I am not on a hosting provider, but a home NAS server and also not publicly sharing anything, so unless Plex is blocking a residential ISP like Bell Canada…which seems unlikely.
Plex’s status page indicates that everything is up. I see other forum posts on issues like Remote Access not working or Remote Access defaulting to port 0 on the public IP address…but those are all a couple of years old and don’t seem to have any relevant similarities.
Anyway, I’m a bit stumped and not sure how to troubleshoot or resolve this.
