Remote connection goes green for 10 seconds then red and stays red. Tried upnp, direct port forwarding and putting the server in the my router’s DMZ. Still no external connection unless I leave relay on. I have downgraded as far as server 1.20 and still no joy.

Generally speaking… ignore the remote access status indicator briefly showing a green icon – that’s just the default when Plex starts testing its status.
Yes, I can reach other let’s of services running on the same NAS while in the DMZ but I can’t ever reach the port that I set the Plex server to run on. Even if I have it use a port that the other service uses after turning that service off the port immediately stopped being open. Almost as if Plex isn’t even presenting an off facing service and only serving interior clients.
QNap NAS, my router (Asus, up to date firmware), old setup that had been working up until about 2 or 3 weeks ago when eventually users reported the issue. Haven’t checked for double Nat yet but nothing else stopped working.
Already tried disabling UPNP as well as putting the NAS in the DMZ. Pretty never showed as open when checking with can you see me but other services on the NAS do report open correctly.
