I removed a couple of users recently (1-4 weeks ago) but I noticed they are still streaming like normal. Why is this? How can I permanently remove them?
They were not managed users. Just a regular friend add. They do not appear under friend/shared users at all.

You need to remove them from authorized devices. Go to Settings > Authorized Devices.
Under authorized devices all I see are a list of devices I use personally.
How can I access the old users authorized devices?
Please PM me the username or email for these users.
Those usernames have now disappeared. However, I now notice “unknown user” has access the server and is playing content regularly!
What is going on!
I’ve never seen “unknown user”. Can you provide me the PMS when this occurs and let me know about what time so I can see who it is?
Ok, I restarted my server and PMS and I believe it had not registered/listed that user under viewed played history.
Both issues have somewhat resolved themselves! 
I’m having the same issue, and what makes it worse is that Plex originally wasn’t tracking their live streaming, but after updating this evening, I don’t know what changed, but now it is. This is seriously unacceptable!
FYI; MovieFan.Plex, I PM’d you just before I realized that it was now tracking her streaming live.
