The very first entry in 'recently added in TV Shiows is years old and wrong, the other entries are all correct and accurate, but the first one seems ‘pinned’ there and won’t go away.
The real recently added begins at the second icon, the first just seems stuck there. The file is indeed an old file on disc i dont know why the metadata thinks its recently added.
Change the sorting order of your library view to “added at”.
Now look at the very beginning or end of the list.
You may find that the item in question has an “added at” date in the future.
Perform the Plex Dance with it to fix.

You actually led me down the right path, the actual file on disk had a “last modified date” of 2098, which is why it was always the latest file!
so not a plex issue at all!
