For a couple weeks now Revision 3 isn’t working. I can see all the videos but when I try to play them I get errors. I’ve tried playing the videos on iOS and PlexWEB. Anyone else having issues with Rev3?

This issue has been addressed already (this commit to be exact), but updates to the Services bundle are currently released with updates of Plex Media Server.
