It all works flawlessly on my 2012 model, with the proxy on a Synology docker (no special parameters required).
Installing the plex app package with SammyWidgets indeed requires for the archive to be zipped without a “plex” folder, as indicated above by sanelyidle, but it is all very straightforward otherwise.
FWIW, I got my app working again by changing Plex server’s network connections to “Preferred” and removed the checkmark next to "Disable weak TLS versions ". In Orca’s V.2013 app -->preferences -->system signed out of Plex account and disabled secure connections and manually added the lan server IP to “my Servers” tab.

After several months I must connected an USB Drive to my Samsung SmartTV 2012 Eseries to watch any content with Plex, now all my problem has done with this solution. I installed the new @Orca 's Plex for Samsung 2.014 in two of my 2012 SmartTV. And it working flawlessly. Use this method to install the new Plex for Samsung SmartTV model 2012. I had try to install new Plex for Samsung from @orca with Sammy Widget, but never succeed. It always show “Failed to Install” error message. But when I try the Proxy server method I mention the link above, the installation working flawlessly in the first try. I repeat the process with my other Samsung SmartTV 2012 model in my bedroom. And again, it working with a charm. Maybe you should try the Proxy server method.
