Direct streaming is fine. Other bit rate files are fine. Disable hw transcode and it is fine. Completely repeatable.

jellyfish-40-mbps-hd-hevc.mkv (HEVC/MAIN/4.1/HIGH) works fine.
jellyfish-45-mbps-hd-hevc.mkv (HEVC/MAIN/4.1/HIGH) works fine.
jellyfish-50-mbps-hd-hevc.mkv (HEVC/MAIN/5.0/HIGH) fails to start.
jellyfish-55-mbps-hd-hevc.mkv (HEVC/MAIN/5.0/HIGH) works fine.
jellyfish-60-mbps-hd-hevc.mkv (HEVC/MAIN/5.0/HIGH) works fine.
The file does not appear to be corrupt, MKVtools doesn’t indicate an issue, and it plays locally just fine.
Not just this server/client version, I have seen this error for months of revisions.
Interesting. I remuxed it through MKVtoolNIX, it didn’t report any issue, nor did the resultant file work in plex. It did work (similarly to the origional) in MPV player and in latest VLC.
care to share any details on how you perform intestrety checking?
PS I just tried it again with Version 1.16.0.1226, unchanged.

thx for the hints. I tried both, results unchanged. Interesting that you found an issue with the 50mbit sample while I didn’t. I downloaded it again and tried once more but it still came up clean. Maybe it is a different version of FFMPEG?
