Could you make the following changes to the firewall
In Allowed Programs - please add PexScriptHost.exe which is in same folder as Plex Media Server.exe

Separately in Advanced Settings , Inbound Rules, setup an Inbound Port Rule as follows
Protocol: TCP
Local Port: 32400
Remote Port: Any
Remote IP: Any
Scope: All Profiles
Action: Allow

Restart the system after making the changes

Does Avast have any firewall type rules as well ? If it has please check and create rules.

If problem persist, get logs and screenshots of all the firewall and related AV settings

Lets wait and see the outcome with the firewall changes. If the timeouts still can be seen in the plex media server agent/plugin logs then would want to try running without the vpn application

Ok issue has still there although a lot less frequent.

In answer to your questions:
What was the interval set to before? - Whenever a change is detected
Could you make the following changes to the firewall? - Done I think. Already had a plex script host rule but added one again? Also done other changes.

Have restarted lets wait and see

The logs suggest you also had periodic library updates before

you only mentioned Dlna server and media server. Remember program rules in the firewall have no ports specified and port rules in the firewall have no programs specified.

Any specific rule in avast ?

The frequency may have been 15 mins before.
The send debug to server is definetly selected on the TV but I can see the logs in the attached to this comment.
No firewall as part of Avast. Still happening around 21:50 tonight

At 21:44 there was a Refresh of the Kids TV Shows library

Oct 23, 2016 21:44:52.137 [0336] DEBUG - Request: [127.0.0.1:64578 (Loopback)] GET /library/sections/2/refresh (4 live) GZIP Signed-in Token (antcull)

Was this manually instigated through the Refresh button ?
How many media items were added since last refresh?

At the moment the database is optimized whenever we add what is deemed to be significant to warrant a database optimization. Unfortunately the database is locked out whilst it is optimizing and timeouts may arise

Was this manually instigated through the Refresh button ? No
How many media items were added since last refresh? No idea content is added automatically

Why would these issues only have started in the last few weeks? I have been running plex in the same way for over a year.
