I’ll scrub and provide any logs required, but as far as I can tell this seems to be an audio transcode that’s failing. Most of what I see this on is direct streaming video, but transcoding audio down to stereo.
The server is currently running on Windows Server 2012 R2.
It’s a Xeon E5-2650 v3, 16GB RAM, nVidia P400 for video transcodes.
